Job Overview:

The Family Navigator will assist in collecting information on family social needs, barriers, and issues at Children's Specialized Hospital. The main responsibility is to conduct Social Determinants of Health (SDOH) screenings with patients and their families, review patient data, and identify ways to assist families in need. The Navigator will engage with families, maintain confidentiality, provide information, and communicate social needs to a multidisciplinary team. Additional duties include data collection, monitoring, analysis, and other tasks as assigned. Remote work is available, with occasional travel to CSH sites.

Qualifications:

Required:

  • High school diploma

Preferred:

  • Spanish bilingual

Scheduling Requirements:
