Job summary

Pharmacyupdate is seeking a Clinical Case Manager to join the Complex Care team. This full-time role involves coordinating patient care plans and navigating complex pathways, with hybrid working arrangements requiring 2 days in the Bournemouth or Stockport office each week.

You will work closely with clinicians to ensure timely access to services, monitor patient progress, and support multidisciplinary collaboration to optimise outcomes for complex cases.

Qualifications
