Responsibilities

• Provide clinical leadership across supported living services.
• Review and implement care plans for individuals with complex/high risk needs.
• Act as the lead clinical liaison for referrals, transitions and MDT meetings.
• Support service managers to embed best practice and maintain CQC compliance.
• Monitor incident reports, safeguarding alerts and health outcomes.
• Lead on health audits, clinical risk assessments and medication reviews.
• Train and mentor support staff in traumainformed, high‑quality care.
• Collaborate with local authorities, NHS commissioners and families.
