Job summary

Responsibilities

• Conduct assessments to identify needs during transitional phases.
• Develop and implement tailored support plans with families and multidisciplinary teams.
• Provide direct support and guidance to children and families, ensuring voices are heard.
• Monitor and review progress, adjusting support plans as needed.
