Job summary

Hywel Dda University Health Board in Carmarthen is seeking a Band 3 family support worker to join the Children’s Disability Health team. You will support children and young people up to age 18 with disabilities and their families to reach their full potential and equity of opportunity within their community.

You will work with colleagues to assess, plan, and deliver care, manage your own schedule, and may conduct home visits after 5 pm to meet family needs, based at Hafan Derwen, Carmarthen.

Qualifications

• Welsh language desirable.
• Experience in children’s disability services preferred.
• Ability to do home visits after 5 pm if required.

Responsibilities

• Work as part of the children’s community team delivering care to children/young people with care needs and their families in home, community or educational settings with supervision by a registered healthcare professional.
• Undertake personal and clinical care duties with minimal supervision in line with HEIW All Wales Guidelines for delegation.
• Keep healthcare professionals regularly updated on patient condition and progress.
• Promote a healthy, safe environment for babies, children and young people and ensure infection control policies are followed.
