The role entails case managing and caring for children with chronic or acute nursing needs outside of hospital settings, within a nurse-led team. It offers an opportunity to share ideas and improve patient outcomes.
The service operates from 9-5, seven days a week. This is a full-time position, offering 37.5 hours per week, with some weekend and bank holiday work required, to cover maternity leave.
We seek a motivated, forward-thinking registered children's nurse with strong professional knowledge and excellent communication skills. The ability to prioritize and work independently or as part of a team is essential.
You will provide high-quality clinical and non-clinical care, outcome-focused, supporting children and families to participate fully in life at home and in the community. The workload is varied, with opportunities for further development.
You will also support junior team members through skill development and staff appraisals, teach parents, carers, and health professionals, and mentor students. You will work within a supportive nurse-led service, collaborate with clinical nurse specialists, gain management experience, and work closely with Consultant Paediatricians to plan, implement, and evaluate care.
