Job summary

Coventry and Warwickshire Partnership Trust is seeking a clinician to join the CEN team within the Community Mental Health pathway. You will manage a smaller caseload, undertake assessments, coordinate clinical care and develop bespoke formulation-led interventions with supervision from the MDT.

You will collaborate with Community Mental Health Teams to ensure plans are understood and implemented consistently, while maintaining high standards of care and timely clinical records.

Qualifications

• Manage a smaller caseload to provide intensive, responsive support and interventions.
• Undertake assessments, coordinate clinical care, respond to emerging needs.
• Develop bespoke formulation-led care plans with positive risk management.
• Coordinate with the Community Mental Health Team to ensure plans are understood and implemented consistently.
• Participate in supervision and reflective practice within the CEN pathway.

Responsibilities

• Clinical Work: Manage a smaller caseload to provide intensive, responsive support and interventions.
• Undertake assessments, coordinate clinical care, respond flexibly to emerging needs.
• With supervision from the MDT, develop formulation-led care plans and positive risk-management plans.
• Coordinate with the wider Community Mental Health Team to implement plans consistently.
• Professional Responsibilities: Practise under your professional code, maintain accurate records, promote safe, compassionate, person-centred care.
• Training, Reflective Practice and Supervision: Participate in supervision and reflective practice within the CEN pathway.
• Teamwork: Work with psychologists, CEN Clinical Leads and Wellbeing & Recovery Facilitators and CMHTs to promote coordinated care.
