Job summary

West London Mental Health Trust is seeking a Band 6 Practitioner to join the Complex Emotional Needs (CEN) Service, covering Ealing, Hounslow and Hammersmith & Fulham. The role supports adults with complex emotional needs in a tier 3 community setting and involves caseload management within a multidisciplinary team.

You will screen referrals, undertake assessments, develop care plans and provide therapeutic interventions, while contributing to service development and effective pathways of care.

Responsibilities

• Screen and assess referrals and carry out comprehensive assessments and formulations.
• Develop individual care plans and provide therapeutic interventions for a caseload of clients.
• Liaise with referrers and work closely with a multidisciplinary team to ensure joined-up care.
• Contribute to service development, quality improvement and new ways of working within CEN.
• Collaborate with others to deliver compassionate, person-centred and evidence-informed care.
