Qualifications

• Experience in a supported housing or support work setting.
• Strong experience in support planning and key working.
• Understanding tenancy-related support and sustainment.
• Experience supporting clients with complex needs including substance misuse.
• Ability to work independently in a lone-working environment.
• Excellent organisational and caseload management skills.

Responsibilities

• Manage a caseload of clients, providing ongoing tailored support.
• Deliver effective key working sessions focused on individual needs.
• Support clients with benefits, tenancy sustainment, and independent living skills.
• Work with individuals who may have complex backgrounds including substance misuse.
• Develop and implement support plans and risk assessments.
• Maintain accurate case records and documentation.
• Work across multiple service locations within Hounslow (flexibility for travel)
