Job summary

Hampshire County Council is seeking a Case Worker to join the Older Adults Team. This role provides individuals aged 65 and over with support and guidance to maximise independence and wellbeing.

You don’t need prior social work experience; full training and supportive supervision will be provided by qualified social workers and health care professionals. You will develop essential skills while helping people stay independent in their own homes.

It’snot essential that you havepreviousexperience in social work.What’simportant is that you can work compassionately with individuals aswe’llprovide you with support and training from experienced colleagues, qualified socialworkersand health care professionals.

Whatyou’llbe doing:

• Completingassessments and reviews of individuals in the community.
• Providing support and guidance to individuals to maximise their wellbeing and independence.
• With support and training,you’llcollaborate with individuals, their families, providers, and other healthcare professionalsto findand signpost toinnovative, alternative community resources to meet individuals’needs.
