Job summary

Community Care Live is looking for a Case Worker to join our Older Adults Team. You’ll support individuals aged 65 and over with assessments, guidance and activities to maximise independence and wellbeing.

No formal social work experience is required; training will be provided by qualified staff. You will work compassionately in the community, building links with families, providers and other health professionals to identify local resources and signpost to suitable services, with opportunities

• Effective organisational and prioritisation skills.
• Clear written and verbal communication.

It’snot essential that you havepreviousexperience in social work.What’simportant is that you can work compassionately with individuals aswe’llprovide you with support and training from experienced colleagues, qualified socialworkersand health care professionals.

Whatyou’llbe doing:

• Providing support and guidance to individuals to maximise their wellbeing and independence.
• With support and training,you’llcollaborate with individuals, their families, providers, and other healthcare professionalsto findand signpost toinnovative, alternative community resources to meet individuals’needs.
