City Health Care Partnership CIC in Bridlington is seeking a Community Case Manager to coordinate and deliver personalised home-based care, supporting patients with complex health needs and long-term conditions.
You will assess and plan care, develop care plans, and work with GPs, community services and other health professionals to prevent avoidable hospital admissions while promoting safety, independence and quality of life.
Working with our community nursing team, you’ll coordinate and deliver personalised care that helps people stay well and remain independent at home. You’ll play a key role in supporting patients with complex health needs, long term conditions and palliative care, helping to improve outcomes and reduce avoidable hospital admissions.
As a Case Manager, you will manage a defined caseload and work closely with patients, carers, General Practitioners, community services and other healthcare professionals. You will use your clinical expertise to assess needs, develop care plans and provide high-quality nursing interventions, while supporting colleagues and promoting best practice.
