Your role

Rethink Mental Illness is seeking a Passionate& Committed Carers Coordinator

The Carers Coordinator will oversee the provision of support available to carers, friends, and family of mental health patients, who are transitioning from a secure care setting to community with guidance, information, and support. I will develop and build a network of connections and links to appropriate services and support networks throughout the East Midlands. I will work with the IMPACT hub and community teams to improve the provision of services, including developing and facilitating education and peer support groups, to enable carers and families to improve their knowledge and understanding of factors around mental health affecting their loved ones and to support carers to recover their quality of life.

KeyResponsibilities

• I lead and develop on the carers service provided including carers groups, service user forms and other initiatives.

• I have a thorough understanding of how the carers service should be delivered, actively monitoring activities against contractual obligations and ensuring the service demonstrates positive outcomes and impacts.

• I ensure that these is a coordinated and consistent approach to service provision in line with allocated resources.

• I ensure that robust systems and local operating procedures are implemented and understood.

• Attend multi-disciplinary meetings relating to referrals, patients or carers and families in service.

• I will provide information, support, and guidance to the Family Liaison Officer and Support Workers to enable them to better support carers, friends and family of patients transitioning from secure care hospital settings back into their communities across the East Midlands.

• I ensure that all support is delivered in a person-centred way which meet specific outcomes using support planning and outcome measurement tools to document achievement against KPI’s.

• Be a positive role model and coach, providing expertise and guidance on issues affecting carers and their rights to the team; developing their knowledge and skills and embedding the Triangle of Care Approach
