Job summary

The MioCare Group is recruiting a Transfer of Care Officer to join the Community Reablement team in Oldham. The role supports referrals and triage after discharge from acute care, coordinating placements to best meet patient needs.

You will work with a range of professionals seven days a week to promote independence while meeting CQC and MioCare policies. Salary is £14.72/hour with enhancements for unsociable hours.

Qualifications

• Experience in health or social care settings preferred.
• Ability to triage, prioritise and coordinate referrals.

Responsibilities

• Support priority of care after clinical triage and manage referrals within set parameters.
• Manage a designated caseload to enable discharge to assess and community enablement.
• Advocate for patients to ensure safe, timely discharges and empower self-care.
• Liaise with acute, community and partner organisations to improve access to services.
