Job summary

Achieve together Limited is seeking a Team Manager in Grantham, Lincolnshire, to lead a care team across home and community settings. You will oversee care planning, ensure high-quality person-centred support, manage records, and support regulation-compliant services.

The role involves coaching staff, ensuring safe staffing levels, and contributing to recruitment and development while maintaining strong relationships with families and colleagues.

Qualifications

• Experience in health, social care or supported living.
• Experience leading, supervising or coordinating a team with clear direction.
• Experience coaching and developing team members.
• Strong understanding of person-centred care and care plans.
• Knowledge of safeguarding and reporting concerns.
• Understanding of regulatory standards for the service.
• Experience maintaining accurate, confidential records.

Responsibilities

• Lead and coach the team to deliver high-quality, person-centred care.
• Oversee care plans in home and community.
• Maintain up-to-date records, diaries and reports.
• Ensure safe staffing and rota management.
• Support with supervision, appraisals, and performance reviews.
• Assist with recruitment and staff development.
• Work with the Registered Manager to meet standards and budgets.
• Collaborate with relatives and Advocates.
