Job summary

Peterborough City Council is seeking a dedicated Care Support Worker to join the Reablement Team. You will support clients to improve wellbeing and independence, monitor progress, and conduct reablement assessments through their journey.

The role runs Monday–Friday, 9-5, with no weekends; flexibility may be required. A full UK driving licence and access to a vehicle are essential. Interviews are 28th/29th September at Sand Martin House.

Qualifications

  • Excellent interpersonal and organisational skills are essential.
  • Knowledge and/or experience of the needs of vulnerable adults.
  • Understanding of the Care Act, Eligibility Criteria and Mental Capacity Act is essential.
  • Full UK driving licence and access to a vehicle is required.

All about the Role

Peterborough City Council's Reablement Team is seeking a motivated and caring individual to join the team as a Care Support Worker.

In this role, you will help maximise individuals' wellbeing and promote their independence by supporting and monitoring clients throughout their reablement journey.

You will work with clients to support them through their reablement time. You will also work with colleagues to support individuals to achieve their goals and complete our Reablement support workers assessments.

As a Care Support Worker, you will collaborate with colleagues and partner agencies to deliver effective reablement support. You will also undertake Adult Social Care assessments to identify longer-term care and support needs, and carry out six-week reviews to evaluate progress and ensure appropriate ongoing support is in place.

This role involves working Monday - Friday 9-5, with no weekend work!!Please note - Occasionally you will need to be flexible with start and finish times to fit the needs of the service.Interviews will be held on 28th and 29th September at Sand Martin House
