Job summary

Age UK is seeking a Care Navigator/Social Prescriber to support Havering residents through the Ageing Well Service. You will work with health, social care and voluntary partners to improve wellbeing, reduce isolation and enable people to access community resources.

You will maintain client records, support monitoring and evaluation, and contribute to service development while providing compassionate, person-centred care to older adults and those with long-term conditions.

Qualifications

• Experience working with older people or those with multiple long-term conditions.
• Excellent communication and ability to work effectively both one-to-one and in a team.
• Good computer skills and accurate record keeping.
• Enhanced DBS Disclosure required for this post.

Responsibilities

• Provide Care Navigation support to Havering residents with multiple long-term conditions and social needs.
• Maintain client records and support monitoring and service evaluation.
• Collaborate with health, social care and voluntary sector to increase reach and improve wellbeing.
