Job summary

Qualifications

• Substantial experience as a social prescriber in health, social care, or voluntary sector, with at least two years of direct engagement.
• Experience working with GP practices and community teams within a PCN.
• Excellent interpersonal, communication, and organisational skills; able to work in diverse environments.

Responsibilities

• Support older people living with frailty to live longer, healthier lives.
• Coordinate care with PCN GP practices, local authority, and voluntary sector.
• Act as single point of contact for patients and carers.
• Promote social prescribing and improve access to services.
• Maintain accurate records and collaborate with multi-disciplinary teams.
