NHS is seeking two part-time Care Navigators to enhance patient care by providing a welcoming service and assisting patients in accessing the right healthcare. Ideal candidates will possess strong communication skills and have a background in primary care, making a tangible difference in community health.
We're looking for two part time friendly, proactive Care Navigators to be the welcoming face of our practice and the vital first point of contact for our diverse patient community. In this role, youll support patients in accessing the right care at the right time, working closely with our clinical and admin teams to help deliver high-quality, compassionate services.

Act as the first point of contact for patients, both inperson and over the phone, providing a welcoming and professional service
Signpost patients to the most appropriate service orhealthcare professional, using practice protocols and clinical triage systems
Manage appointment bookings, cancellations, and queriesusing the practices clinical system
Work closely with GPs, nurses, and other team members toensure smooth coordination of patient care
Handle sensitive information with confidentiality andprofessionalism
Support patients in navigating health and communityservices, promoting self-care where appropriate
Help maintain accurate patient records and contribute to theefficient running of the reception and admin functions

The following are the coreresponsibilities of the Care Navigator. There may be, on occasion, a requirementto carry out other tasks. This will be dependent upon factors such as workloadand staffing levels:
a.Maintain and monitor the practice appointmentsystem
b.Process personal, telephone and e-requests forappointments
c.Answer incoming phone calls, transferringcalls or dealing with the callers request appropriately
d.Signpost patients to the correct service
e.Initiating contact with and responding to,requests from patients, team members and external agencies
f.Code data on the clinical IT system
g.Data entry of new and temporary registrationsand relevant patient information as required
h.Input data into patients healthcare recordsas necessary
i.Direct requests for information, i.e., SAR,insurance/solicitors letters and DVLA forms to the administrative team
j.Manage all queries as necessary in anefficient manner
k.Carry out system searches as requested
l.Maintain a clean, tidy, effective working areaat all times
m.Monitor and maintain the reception area andnotice boards
n.Support all clinical staff with general tasksas requested
