Hampshire County Council is recruiting for Care Leavers' teams to support young people aging out of care. The role involves mentoring and coaching 16-25-year-olds, coordinating with partner agencies, and helping them navigate adulthood.

The position is hybrid, requiring travel and a valid driver's licence with regular vehicle access. You will work within a supportive team, building relationships, managing diaries, and delivering practical guidance to empower independence and positive life

Qualificações

• Experience working with care leavers or similar vulnerable client groups.
• Ability to establish and maintain relationships with young people aged 16-25.
• Comfort with liaising with partner agencies and families/carers.

Responsabilidades

• Hold a caseload of care leavers aged 16-25 from varied backgrounds.
• Organise diary and priorities, visiting young people and completing admin.
• Act as a mentor and coach to support independent living and positive choices.
• Liaise with partner agencies to secure best outcomes for each young person.
• Travel across Hampshire and the Isle of Wight or other local authorities; hold a current driving licence and access to a vehicle.

As corporate parents, we want our young people to grow into confident adults and support them in making the transition into adulthood and independent living; we know that success will look different for each individual.What you'll do: You will be responsible for a case load of young people aged 16-25 from different backgrounds with differing life experiences. You will organise your diary so that you can manage the priorities of visiting young people and complete the administrative work. The main role is to act as a mentor and coach for young people so that you can support them to reach their full potential. This will mean working individually with young people and liaising with our partner agencies to ensure the best outcomes and empower young people to make their own positive choices. The role is hybrid so you will be expected to work on the move, at home and in our offices., You will be required to travel to various locations within Hampshire and the Isle of Wight or other local authority, and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis., We are a Disability Confident Employer - committed to ensuring that our recruitment and selection process is inclusive and accessible.
