Job summary

Hampshire County Council is seeking Personal Advisers to join the Care Leavers team. You will mentor and support young people aged 16–25 to help them transition to adulthood and independent living, working with partner agencies to achieve positive outcomes. The role is hybrid, with duties across homes, offices, and outreach locations.

You will need empathy, patience, and excellent IT and record-keeping skills, with the ability to work both in a team and independently as required.

Qualifications

• Experience of working with care leavers or similar client group is essential.
• Clear evidence of empathy, patience, and compassionate approach.
• Ability to establish relationships with young people aged 16–25 and liaise with partner agencies.
• Strong IT skills and accurate record-keeping using available resources.
• Able to work collaboratively as part of a team and independently when required.

Responsibilities

• Case load of young people aged 16–25 from diverse backgrounds.
• Organise diary to manage visits and complete admin tasks.
• Mentor and coach young people to support their independence.
• Liaise with partner agencies to secure best outcomes.
• Work in a hybrid environment—on the move, at home and in offices.
