Job summary

Linkit Recruitment is seeking an experienced Personal Advisor for a Leaving Care role within a Children's Social Care team in the UK. You will guide care leavers toward independence, offering practical advice and ongoing support to help them transition into adulthood.

Key duties include advising on finances, housing, education and employment, supporting independence, and coordinating Pathway Plans with social workers and other agencies.

Qualifications

• Experience working with children, young people or care leavers.
• Knowledge of safeguarding procedures and challenges faced by care leavers.
• Ability to build positive and supportive relationships with young people.
• Good communication, organisation and record-keeping skills.
• Ability to work independently and as part of a multi-agency team.
• Understanding of promoting independence, resilience and positive outcomes for young people.

Responsibilities

• Provide practical advice and support to care leavers around finances, accommodation, education, training and employment.
• Support young people with developing independence, confidence and decision-making skills.
• Contribute to the preparation, monitoring and review of Pathway Plans.
• Promote positive relationships and facilitate contact with birth family members where appropriate.
• Support young people to access health services and maintain their physical and emotional well-being.
• Assess and identify risks and needs, ensuring safeguarding concerns are reported appropriately.
• Work closely with social workers, partner agencies and other professionals to coordinate support.
• Attend reviews, case conferences and other relevant meetings.
• Maintain accurate and up-to-date case records in line with departmental procedures.
• Monitor young people's progress and provide ongoing support to help them achieve positive outcomes.
