Job summary

Carrington Blake Recruitment is seeking a dedicated Personal Advisor for Children’s Social Care (Care Leavers). You will support care leavers in transitioning to adulthood, providing guidance on housing, budgeting, education and employment, and working with housing providers, health and education partners to coordinate tailored Pathway Plans.

You will monitor progress, maintain case records, and promote independence while safeguarding and confidentiality are upheld across all activities.

Qualifications

• Experience supporting vulnerable young people or care leavers.
• Understanding safeguarding and risk assessment procedures.
• Ability to coordinate complex support with multiple agencies.
• Strong written and verbal communication and record-keeping.
• Commitment to equality and improving outcomes for young people.

Responsibilities

• Provide practical advice on housing, budgeting, education and employment.
• Prepare, monitor and review Pathway Plans.
• Maintain contact to monitor progress and wellbeing.
• Assist access to healthcare and community opportunities.
• Promote independence and healthy relationships.
• Identify safeguarding concerns and report.
• Coordinate with social workers, housing, health, education and other partner agencies.
• Attend case reviews and represent the service.
• Maintain case records including ICS and databases.
• Ensure expenditure is properly authorised and confidential.
