Job summary

Connect2Halton is seeking a dedicated Personal Advisor to join the Care Leavers Team. You will support care-experienced young people as they transition to adulthood, delivering practical, emotional and professional guidance to access education, employment, housing, health and wellbeing.

You will work with partner agencies to implement Pathway Plans, undertake assessments, maintain case records and advocate for the young person’s voice while safeguarding and promoting independence.

Qualifications

• Experience of working with vulnerable children or care leavers.
• Excellent communication and relationship-building skills.
• Sound understanding of safeguarding principles and relevant legislation.
• Ability to manage a varied caseload, prioritise effectively, and maintain accurate records.
• Experience of multi-agency working and developing professional partnerships.
• Person-centred approach promoting independence and positive outcomes.
• Commitment to equality, diversity, inclusion and anti-discriminatory practice.
• Strong organisational skills and ability to work independently and in a team.
• NVQ or the equivalent.

Responsibilities

• Provide an efficient, best-value service aligned with policies and pathway plans.
• Support care leavers in developing practical independent living skills, including budgeting, housing, education, employment and accessing community services.
• Deliver targeted interventions identified through Pathway Plans and assessments, up to age 25 where applicable.
• Build and maintain positive, professional relationships that centre the young person's voice.
• Promote safety, welfare and best interests, responding to safeguarding concerns in line with Council procedures.
• Maintain accurate case records using the Council's electronic systems.
• Work with internal teams and external partners to coordinate support and outcomes.
• Promote equality, diversity and inclusion in all actions.
• Comply with Health and Safety policies and safe working practices.
• Undertake any other duties appropriate to the role.
