Job summary

Qualifications

• Knowledge of safeguarding principles and relevant legislation.
• Ability to empower young people to achieve independence.

Responsibilities

• Provide advice, guidance, and practical support to care leavers to help them achieve independence.
• Monitor Pathway Plans and ensure they are person-centered and reflect individual needs.
• Promote positive relationships with birth families and facilitate contact where appropriate.
