Job summary

Tranquille Care LTD is seeking a compassionate Personal Advisor for Care Leavers to support young people transitioning from care to adulthood. You will manage a caseload, build trusted relationships and coordinate education, employment, housing and wellbeing services in collaboration with families, social workers and partner agencies.

Qualifications

Responsibilities

  • Manage a caseload of young people leaving care, providing personalised support from age 16 to adulthood.

  • Maintain regular contact and case records up to age 21 (25 where entitled).

  • Identify safeguarding concerns and escalate as required.

Job description

We are looking for a compassionate, motivated and dedicated Personal Advisor Care Leavers to make a lasting difference in the lives of young people transitioning from care into adulthood. This rewarding role offers the opportunity to empower care-experienced young people to develop the confidence, resilience and practical life skills they need to achieve independence and thrive.

As a Personal Advisor Care Leavers, you will build trusted relationships with young people aged 16 and over, acting as their key professional throughout their transition to adulthood. Working alongside colleagues, families and partner agencies, you will coordinate tailored support that enables young people to access education, employment, training, suitable accommodation and emotional wellbeing services while ensuring statutory responsibilities are delivered to the highest standard.

Responsibilities of the Personal Advisor Care Leavers Role
  • Manage a caseload of young people eligible for leaving care services, providing personalised support from age 16 through to adulthood in line with statutory requirements.
  • Build positive, trusting and professional relationships that encourage young people to engage with support services and achieve greater independence.
  • Assess individual needs and contribute to the preparation, implementation and regular review of Pathway Plans.
  • Coordinate multi-agency support to ensure young people receive timely and effective services that promote successful transitions into independent living.
  • Support young people to access education, employment, apprenticeships, training and other post-16 opportunities.
  • Provide practical advice and guidance on housing, budgeting, financial capability, welfare benefits and daily living skills.
  • Advocate on behalf of young people, accompanying them to appointments, interviews and meetings where appropriate.
  • Assist young people with CV preparation, job applications and accessing employment opportunities.
  • Deliver one-to-one and group-based interventions focused on life skills, wellbeing and preparing for adulthood.
  • Maintain regular contact with care leavers, ensuring effective tracking and ongoing support up to the age of 21, or 25 where statutory entitlement applies.
  • Work collaboratively with social workers, education providers, employers, health professionals, housing services and voluntary sector organisations.
  • Identify and respond appropriately to safeguarding concerns, escalating risks in line with safeguarding procedures.Maintain accurate case records, reports and documentation in accordance with statutory guidance and departmental standards.
  • Promote equality, diversity and inclusion, ensuring services meet the individual needs of young people from all backgrounds.
  • Participate in supervision, professional development and mandatory training to maintain high standards of practice.
  • Undertake occasional evening work and group sessions to meet the needs of the service.
Skills and Background Required for the Personal Advisor Care Leavers Role

We are seeking an enthusiastic Personal Advisor Care Leavers who is passionate about improving outcomes for care-experienced young people and supporting them to reach their full potential.

The successful Personal Advisor Care Leavers will demonstrate:

  • Experience supporting vulnerable young people, care leavers or children in care within a social care, youth work or related setting.
  • Good understanding of legislation and statutory guidance relating to leaving care services, including the Children Act 1989, Care Leavers (England) Regulations 2010 and associated guidance.
  • Knowledge of the challenges faced by care-experienced young people transitioning into adulthood.
  • Experience developing support plans, assessing needs and coordinating multi-agency services.
  • Strong understanding of safeguarding principles and the ability to identify and respond appropriately to concerns.
  • Excellent communication and interpersonal skills, with the ability to build positive and trusting relationships.
  • Ability to motivate, encourage and empower young people to achieve positive outcomes.
  • Knowledge of education, employment, training, housing and welfare support available to young people.
  • Excellent organisational skills with the ability to manage a varied caseload and maintain accurate records.
  • Strong advocacy, negotiation and partnership-working skills.
  • Commitment to equality, diversity, inclusion and delivering person-centred services.
  • Flexibility to work occasional evenings and travel as required to meet the needs of young people.
