Job summary

Diamond Blaque HR Solutions is seeking a Personal Advisor in Merseyside to support young people leaving care. You will translate person-centred plans into practical steps, helping with accommodation, education, training and employment during a hybrid assignment.

You will assess needs, monitor Pathway Plans, maintain wellbeing, and coordinate with families and partner agencies while keeping records confidential and up-to-date.

Qualifications

• Clear communication and practical, person-centred support.
• Pathway planning and care leaver support.
• Safeguarding awareness and identification of risk and need.
• Multi-agency working and service coordination.

Responsibilities

• Help care leavers take practical steps towards suitable accommodation, financial capability, education, training and employment.
• Assess needs and prepare, monitor and review Pathway Plans that place each young person at the centre.
• Stay in regular contact, supporting young people's wellbeing and confidence as they make decisions about their futures.
• Connect young people with community opportunities and specialist services that meet their needs.
• Identify risks and report safeguarding concerns through the appropriate channels.
• Work with families, council colleagues and partner agencies, including attending reviews and case conferences.
• Keep accurate, up-to-date case records on ICs and other systems, handle information confidentially and record authorised expenditure.
