Job summary
The Care Home Advanced Clinical Practitioner (ACP) will operate as an autonomous, independent and highly skilled clinician within the Primary Care Network. The post holder will lead the Care Home Team in both reactive and proactive care for residents living in care home settings, providing rapid assessment and treatment of unwell residents and supporting GP practices and community teams.
They will drive clinical leadership, governance and education across the service, oversee complex case
Qualifications
- Registered healthcare professional with current registration.
- Awarded MSc in Advanced Clinical Practice or ACP equivalent.
- Independent Prescriber qualification highly desirable.
- Demonstrated advanced clinical assessment and decision-making.
Responsibilities
- Lead and oversee the Care Home Team daily operations.
- Provide clinical leadership for the Care Home Service.
- Drive quality improvement, audits and governance.
- Investigate incidents and safeguarding concerns; implement learning.
- Deliver education and supervision of staff and trainees.
- Collaborate with GP practices and community teams to improve outcomes.
