CCH Group, the UK’s largest and most trusted care provider, is seeking a dedicated Care Coordinator in South Shields to organise and deliver high-quality home care within the community. You will manage care workers, assess and review individual care plans, and supervise field staff.

A valid driving licence and access to your own vehicle are essential due to travel. Typical hours are Monday to Friday, 9am–5pm, with on-call duties on a rota.

Qualifications

• Valid driver’s license and access to own vehicle due to travel.
• Experience in community/home care settings preferred.
• NVQ Level 3 desirable.
• Strong rostering and field-based coordination abilities.
