Job summary

Care By Us is seeking a Care Coordinator to organise and deliver high-quality in-home care within the community. You will manage care workers, assess care plans, and support field staff while maintaining strong relationships with clients and teams.

The role requires a valid driver’s license and access to your own vehicle due to travel, with a Monday to Friday 9am–5pm schedule and additional on-call duties on a rota basis. An Equal Opportunities employer, we value community care excellence.

Qualifications

• Valid driver’s license and access to own vehicle.
• Experience in community care or rotas is preferred.
• Knowledge of safeguarding and care planning is advantageous.

Responsibilities

• Assess and review individual care plans and supervision of care staff in the field.
• Organise and optimise rotas to meet client needs.
• Travel as required to deliver in-home care across assigned communities.
