Care By Us, the UK’s leading home care provider, is hiring a Care Coordinator to organise and deliver high-quality services in the community. You will manage care workers, coordinate schedules and ensure care plans are implemented effectively.

This field-based role requires a valid driving licence and access to your own vehicle. Hours are Monday to Friday 9am–5pm with on-call duties on a rota.

Qualifications

• Efficient, organised Care Coordinator with a passion for community care.
• Experience in managing rotas and building relationships with care workers.
• Healthcare experience and understanding of the Care Inspectorate, care plans, and rotas.

Responsibilities

• Assess and review individual care plans and update them accordingly.
• Supervise, instruct, and support care staff in the field.
• Travel involved; access to own vehicle required due to field duties.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
