The Cinnamon Care Collection is seeking a Care Coordinator to support the General Manager and Deputy Manager in running Earlsfield Court, a 72-bed care home offering luxury residential and dementia care. The role focuses on maintaining high-quality, person-centred care and stepping in for the Deputy Manager when needed.

You will coordinate assessments, support the care team, supervise medication management, and contribute to staff development and quality assurance across the home.

Competenze

• NVQ4/Level 5 in Care Management or equivalent is required.
• Two years of experience in long-term care.
• Plan, implement, evaluate and clearly document care delivery.

Mansioni

• Coordinate the resident assessment process with resident involvement and integrate into care plan.
• Communicate relevant information to achieve a person-centred approach.
• Work hands-on with the team to maintain appropriate levels of care; lead by example.
• Prepare the weekly A&I report and address concerns to improve quality outcomes.
• Oversee Safe Medication Management program with auditing and training needs.
• Assist with supervision meetings and annual performance reviews.
• Ensure staff coverage through scheduling and daily assignments in Deputy Manager's absence.
• Conduct formal procedures with the Deputy Manager (investigations, disciplinaries).
