Care Coordinator Early Intervention in Psychosis Team

The post holder will work as part of the integrated multidisciplinary team, maintaining and co-ordinating timely and appropriate interventions and assessment for a caseload of service users living in the community in line with the relevant clinical pathways as outlined within the EIT operational procedure. Interventions will focus on relapse prevention, improving quality of life and collaboratively working towards recovery.
