Job summary

Greater Manchester Mental Health NHS Foundation Trust in Leigh is seeking a Band 6 Care Coordinator for the Early Intervention in Psychosis Team. The post supports individuals aged 14–65 with First Episode psychosis, working within an integrated multidisciplinary team to co-ordinate timely interventions.

The role focuses on relapse prevention, quality of life improvements and recovery, with flexible working and full-time hours in a NHS setting. Training and professional registration are required.

Qualifications

• Experience of working within a multidisciplinary team to support service users and families.
• Experience/knowledge in risk assessment/management and care planning.
• Experience in effectively managing own caseload.
• Ability to establish good working relationships with a range of providers including GP’s.
• Experience with confidentiality and timely case records.

Responsibilities

• Care coordination within the Early Intervention in Psychosis Team.
• Provide evidence-based group and individual interventions including CBT for psychosis where appropriate.
• Support with medications and regular physical health monitoring.
• Assist service users with employment, education and vocational activities.
• Engage in social activity and physical health promotion, plus support for families and carers.
