Job summary

Health Recruit Network is seeking a Care Coordinator to support the day-to-day delivery of a domiciliary care service in Bristol. This hands-on role acts as the central contact between service users, families, care staff and healthcare professionals.

You will manage care packages, staff rotas and liaise with social workers, GPs and local authorities to ensure safeguarding and regulatory compliance. A minimum of 2 years in domiciliary care coordination is required, along with a full UK driving

Qualifications

• Minimum 2 years experience within domiciliary care coordination.
• Strong rota management and care allocation experience.
• Experience maintaining care records and completing compliance audits.
• Knowledge of CQC requirements and safeguarding responsibilities.
• Proficient with MS Office and electronic care management systems.

Responsibilities

• Coordinate and manage care packages ensuring timely, person-centred delivery.
• Prepare, manage and update daily and weekly staff rotas.
• Allocate care calls based on staff availability, skills and location.
• Arrange cover for sickness, annual leave and unplanned absences.
• Monitor missed visits, late arrivals and changes in service user needs.
• Maintain accurate care records, care plans, risk assessments and MAR charts.
• Conduct audits, compliance checks and support CQC inspections.
• Support staff supervisions and spot checks.
• Liaise with social workers, GPs, district nurses and local authority professionals.
• Escalate safeguarding concerns, risks and compliance gaps promptly.
