Job summary

CCH Group in the UK is seeking a Care Coordinator to organize and deliver high-quality home care in the community. The role involves managing care workers, reviewing care plans, and supporting staff in the field. A valid driving licence and access to your own vehicle are required due to travel.

Working hours are Monday to Friday 9am–5pm with on-call duties on a rota. We offer leadership opportunities, access to experts and recognition through Extraordinary Care Awards.

Qualifications

• Experience & Organisation: You are an efficient, organized Care Coordinator who excels in managing rotas and building strong relationships with care workers.
• Stepping Up? Healthcare experience and understanding of CQC, care plans and rotas are helpful for transition into this role.
• NVQ level 3 is desirable.

Responsibilities

• Manage care workers in the field and coordinate, supervise, and support staff to ensure high-quality home care.
• Assess and review individual care plans to ensure they meet client needs.
• Travel to client locations as required and support the field team in delivering care.
