HCRG Care Group in Lancashire is seeking a compassionate Care Coordinator (Administrator) to join our Care Coordination Hub. You’ll respond to calls and emails, signpost referrals, and help families access the right care promptly.
The role is based between Preston and Burnley, requires good IT skills, and a willingness to travel across hubs. You’ll work with clinical teams to schedule routine appointments and support multi‑agency teams to ensure smooth care coordination.
