Job summary

HCRG Care Group in Lancashire is seeking a compassionate Care Coordinator (Administrator) to join our Care Coordination Hub. You’ll respond to calls and emails, signpost referrals, and help families access the right care promptly.

The role is based between Preston and Burnley, requires good IT skills, and a willingness to travel across hubs. You’ll work with clinical teams to schedule routine appointments and support multi‑agency teams to ensure smooth care coordination.

Qualifications

• Excellent communication and customer service skills.
• Calm and supportive when handling high volumes of calls or complex queries.
• Highly organised with strong attention to detail.
• Enjoys a busy, fast-paced team environment.
• Confident using IT systems and keen to learn new processes.
• Willing to travel across Lancashire to support other hubs when required (UK Driving Licence essential).
• Experience in healthcare or coordination roles is desirable but not essential.

Responsibilities

• Provide a friendly, efficient and professional response to all incoming calls and emails into the Care Coordination Hub.
• Filter, prioritise and direct queries to the correct teams.
• Support clinical teams by booking and scheduling routine clinic appointments.
• Ensure referrals and enquiries are signposted promptly within required timescales.
• Work collaboratively with colleagues across the service to ensure smooth coordination of care.
