Job summary

Autumn Search Limited is seeking a dedicated Care Coordinator in Woking, Surrey to join a growing home care provider. The role focuses on coordinating day-to-day care delivery, crafting rotas, and arranging cover while safeguarding clients and staff.

A base of 9am–5pm, Monday–Friday, with an on-call weekend rotation offers progression into leadership within our expanding team. You’ll work closely with the Registered Care Manager, liaising with clients, families, and professionals to ensure

• Experience coordinating care and rota planning in a home care setting.
• Exposure to scheduling, arranging cover, or coordinating care staff is advantageous.
• Willingness to develop in a growing home care provider environment.

Responsibilities

• Coordinate day-to-day delivery of care alongside the Registered Care Manager.
• Assist with preparing rotas and scheduling, including arranging cover when required.
• Support safeguarding procedures and build relationships with clients, their families and care workers.
• Liaise with Social Workers, GP practices and NHS teams as needed.
• Provide admin support and contribute to day-to-day coordination.
• Deliver personal care in the community when required and participate in on-call rota.

What You'll Be Doing

Working alongside the Registered Care Manager, you'll help coordinate the day-to-day delivery of care, including:

• Assisting with preparing rotas and scheduling, including arranging cover when required
• Helping to ensure the well being of both clients and care staff
• Supporting safeguarding procedures
• Building positive relationships with clients, families and care workers
• Liaising with Social Workers, GP practices, NHS Discharge Teams and other professionals
• Supporting the Registered Care Manager with administration and day-to-day coordination
• Providing personal care in the community when required
• Participating in a weekday &1-in-4 weekend on-call rota
