Job summary

Routes Healthcare in Rochdale is seeking a Care Coordinator to schedule and coordinate care visits to ensure our clients receive high-quality care at home.

You will work with centralised support teams and our in-house scheduling system, manage on-call duties, adapt to changes, and help deliver a compliant, compassionate service.

Qualifications

  • Organisational and time-management abilities required.
  • Excellent communication skills essential for coordinating care teams.
  • Understanding of CQC regulations is advantageous.
  • NVQ in Health and Social Care is a bonus.
  • Full UK driving licence with access to own vehicle is required.

Responsibilities

  • Co-ordinate delivery of care using the scheduling system.
  • Adapt to schedule changes and manage staff availability.
  • Ensure healthcare workers attend meetings, trainings and supervision.
  • Assist the Registered Manager to deliver high-quality care.
  • Manage on-call duties on a rotating basis.

Your Typical Day To Day Will Include
  • Co-ordinating the delivery of care using our care scheduling system
  • Adapting to changes in schedules, including staff availability and new clients into existing care runs
  • Ensuring that healthcare workers attend staff meetings, supervisions, and training within CQC-required timescales.
  • Assisting your Registered Manager to deliver a high-quality care service
  • Manage on-call duties on a rotating basis
