Job summary

Care Santé Limited in Hull is seeking a Care Coordinator to support our Hull home care service. The role demands excellent organisation, communication and supervisory abilities, with a salary of £27,113.14 per year and a 37.5‑hour week.

You will work closely with care workers, coordinate rosters, support assessments, and assist in recruiting while participating in the on‑call rota. A level 3 qualification or equivalent, and a driving licence are preferred.

Qualifications

• Must have car drivers licence and own transport.
• Experience in health and social care preferred.
• Level 3 qualification or equivalent preferred.

Responsibilities

• Plan care calls and rosters using UniqueIQ (advance planning 1–2 weeks).
• Facilitate customer care assessments and reviews.
• Collaborate with Local Authority and potential customers to win contracts.
• Oversee frontline staff supervision, spot checks and competency assessments.
• Support recruitment by interviewing care worker applicants.
• Create new care staff users on UniqueIQ and new customer users on UniqueIQ.
• Assist carers with mobile phones (setup, monitoring, support).
• Support with medication checks, transcription and e‑MAR audits.
• Complete ECM reconciliations (CM 2000 and IQ Timecard).
• Be a consistent point of contact for customer queries.
• Maintain training matrix and evidence in UniqueIQ.
• Participate in on‑call rota (paid on top of salary).
• Undertake other office duties as required.

• Participating in the on-call rota is a requirement for the role (1 in 4 weeks, payment is paid on top of salary £270 for the week)

As a Care Coordinator Your Duties Will Include

• Planning care calls and producing efficient daily care worker rosters using our UniqueIQ system. The advance roster must be created in advance of a minimum of one week, ideally two weeks in advance.
• Facilitating customer care assessments and reviews.
• Close cooperation with The Local Authority and potential customers to gain new contracts, and all tasks relevant to the enquiries.
• Supervisory responsibility for frontline staff including care worker supervisions, on-site spot checks and competency assessments
• Supporting recruitment by interviewing care worker applicants
• Crating new care staff users on the UniqueIQ
• Crating new customers users on the UniqueIQ
• Assisting carers with mobile phones. (set up, monitoring, support).
• Supporting with medication checks, transcribing and e-MAR chart audits
• Completing ECM system reconciliations (CM 2000 and IQ Timecard)
• Being a consistent point of contact for customer queries and issues
• Maintaining the training matrix and evidence within UniqueIQ
• Participating in the service’s on-call rota (this is paid on top of salary, one in four weeks).
• Undertaking other office duties as required
