Job summary

City and County Healthcare Group Ltd is hiring a Care Coordinator to organize and deliver high-quality home care in the Grimsby area. You will manage care workers, oversee field-based duties, and ensure plans are implemented to meet individual needs.

A valid driver’s licence and access to your own vehicle are required due to travel. Monday to Friday 9am-5pm with on-call duties on a rota. NVQ Level 3 is desirable, with opportunities for career growth and recognition.

Qualifications

Salary £25,364 + extra payments for on call

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.

Qualifications
