Salary: £27,800 raising to £28,650 after successful completion of probation

Location: Working from both our Warwickshire office near Stratford-upon-Avon & our Kenilworth office covering Coventry

Requirements: Must drive & have own car

Unique Senior Care is seeking a Care Coordinator to join our friendly team, coordinating for our Warwickshire & Coventry branches. Responsibilities include scheduling care hours, matching clients & Caregivers to ensure high-quality, person-centered care. NVQ Level 3 in Health & Social Care or working towards it is preferred.

About Us

Unique Senior Care provides bespoke home care services, mainly supporting the elderly to maintain independence in their own homes.

Responsibilities
  • Ensure all regulatory and contractual standards are met and exceeded.
  • Provide leadership and management to a team of caregivers, fostering professional development.
  • Assess current clients and plan high-quality, person-centered care.
  • Represent the organization effectively in partnerships with local NHS, social work, and community contacts.
  • Support the Registered Manager with care plan writing.
  • Business development and community outreach.
  • Communicate effectively with caregivers, clients, families, and stakeholders.
  • Manage caregiver schedules, including leave requests.
  • Manage client schedules and ensure care plan outcomes are met.
  • Match Caregivers and Clients based on skills and needs.
  • Maintain accurate records on our management system.
  • Interview caregiver candidates.
  • Maintain compliance with legal and company requirements.
