The Ross Practice is seeking an Administration Post to coordinate care for patients with long-term conditions, frailty and complex needs. You will help link patients with health and social care services, minute multidisciplinary meetings, and arrange reviews, while maintaining accurate records and supporting QOF compliance.

This role requires strong organisation, IT literacy, and a compassionate, patient-centered approach.

Qualifications

• Proven ability to communicate clearly with patients and colleagues.
• Experience coordinating care across health and social care services.
• Good IT literacy and experience with basic data handling.

Responsabilités

• Coordinate patient care across health and social care services, ensuring continuity for complex needs.
• Minute MDT meetings and weekly care home meetings.
• Monitor MDT actions and liaise with GPs, nurses, social prescribers, and community teams.
• Invite and book reviews for long-term conditions and frailty.
• Contact patients to arrange appointments and follow-up care.
• Support self-referral and signpost carers to services such as social prescribing.
• Ensure data accuracy and participate in data quality checks and audits.
