Job Role Overview

The role is to support the delivery of care services by carrying out a range of functions in the field, including the assessment and review of individual client services and the practical supervision, instruction, and support of our carers.

Key Responsibilities

• Providing full support to our team of Care Givers.
• Working closely with the Care Manager to coordinate care services and ensure the right Care Giver and client match.
• Liaising with family members and relevant healthcare professionals regarding client care.
• Assisting in planning, coordinating, and scheduling client support to ensure continuity of service.
• Supporting Care Giver training activities.
• Managing daily medication and task prompts.
• Following up on alerts to ensure timely service delivery.
• Managing and recording Care Giver annual leave requests.
• Assisting with Care Giver recruitment.
• Dealing promptly with queries, referring to relevant teams as needed.
• Building and maintaining effective communication with clients, relatives, Care Professionals, and other professionals.
• Writing care plans for new clients.
• Supporting local community and business events and activities.
• Ensuring compliance with quality standards and CQC regulations.
• Reporting recruitment needs to meet client requirements.

Our Care Coordinator will divide their time equally between community office and field activities.
