What we offer

This service has been operating in York for several years and is highly reputable within the community. We support and collaborate with the NHS and York Council to enable people to live independently at home longer.

Job Description
What you’ll do

Being a Reablement Health Care Worker is rewarding. You won’t be caring for people in a healthcare setting but helping them manage their personal care at home. By focusing on abilities and outcomes, you assist them in improving long-term independence.

The role involves supporting individuals discharged from hospital or those who have experienced illness, crisis, fall, or change in circumstances. Our goal is to help individuals gain confidence by working alongside them, sharing in their pride as they achieve goals and independence.
