Job summary

Wight Healthcare seeks a candidate to join the Single Point of Contact team in Fareham, supporting referrals for community services to prevent hospital admissions. Qualifications

• Take referrals from clinicians and patients for community services intervention and support to prevent hospital admission.
• Complete referral forms accurately with up-to-date information for clinician review.
• Liaise with GPs and other health professionals to coordinate action plans.
• Negotiate with multiple therapies/agencies to ensure timely multi-disciplinary assessments.

Responsibilities

• Cover the Single Point of Contact referral service.
• Ensure referrals are processed and documented accurately.
• Coordinate with teams to deliver timely patient assessments.
• Maintain right to work in the UK; no Skilled Worker visa sponsorship available.
