Humans Cornwall is seeking a dedicated Support Broker in Hayle to help individuals access personalised support, information and services, enabling independence and choice. You will manage a caseload, collaborate with professionals and organisations, and coordinate tailored solutions for health, social care and community needs.

Qualifications

Responsibilities

• Design and implement personalised care plans with clients.
• Advocate for clients with external organisations to ensure needs are met.
• Maintain regular communication with clients, progressing cases from referral to closure.
• Coordinate services such as housing support, welfare benefits and independent living aids.
• Attend and represent the organisation in multidisciplinary team meetings.
