About the role

An exciting opportunity has arisen for a Cancer Pathway Coordinator within the 2WW Department. This pivotal role supports the effective management of cancer patient pathways, ensuring timely access to diagnostics and treatment. The successful candidate will play a key part in coordinating Multidisciplinary Team (MDT) meetings, tracking patient progress, and ensuring data is accurately recorded and acted upon to maintain compliance with national cancer targets.

Key Responsibilities

  • Prepare for and attend MDT meetings, recording clinical outcomes in real time.
  • Record actions and ensure they are communicated to the relevant stakeholders, including GPs, within 24 hours.
  • Proactively manage cancer pathways to ensure treatment is delivered within nationally set targets.
  • Monitor individual patient journeys, identifying delays and taking corrective action where needed.
  • Escalate any deviations from the expected pathway to the Executive Cancer Lead, as per the escalation policy.
  • Rebook and reprioritise appointments to maintain pathway continuity, in direct communication with patients.
  • Work with service areas to facilitate appointment scheduling, diagnostics, and treatments.
  • Reporting for Cancer Waiting Times Data Collections, COSD Core & COSD Pathology dataset.
  • Reporting on LQR’s in line with Schedule 4&6 of the NHS Contractual Requirements
  • Work in partnership with MDTs and improvement initiatives to streamline the patient journey.
  • Participate in audits and patient satisfaction surveys to inform service enhancements.

What we need from you

  • Experience with the Somerset Cancer Register
  • Familiarity with clinical systems and medical terminology.
  • Knowledge of patient confidentiality and data protection.
  • Proficiency in Microsoft Office and patient administration systems.
  • Experience in data collection, validation, and reporting.
  • Experience working alongside clinical teams.
