The Role
We are seeking a dedicated and experienced Brokerage Assistant to join our Personalised Commissioning Team. The Team dedicated to ensuring people are receiving the support and care they need to maintain their independence and wellbeing, we are responsible for sourcing packages of care and support from the market place for people who may be funding services themselves or eligible for Health or Social Care funding, the services include but are not limited to Care Homes, Domiciliary Care, Supported living, Extra Care, Reablement, Day Services.
In this role
As the first point of contact for the team, this role will primarily require the individual to triage calls and where possible resolve the query, manage and respond to enquiries into the team inbox and distribute referrals to relevant Brokerage Advisor. The role will include liaising with individuals and their families, internal and external partners and professionals, hospitals across the area and colleagues in the integrated care board. This is a fast-paced role and will require the ability to prioritise and organise effectively.
